Acute disseminated encephalomyelitis (ADEM) in children: a case report

Case presentation: Female, 2 years old, admitted in emergency with fever and backache, evolving abnormal sleepiness. At hospital admission, disoriented, ataxic sleepiness, without signs of meningeal irritation. Encephalitis is suspected a lumbar puncture was performed. Analysis cerebrospinal fluid (CSF) excluded infectious causes central nervous system. Brain magnetic resonance (MRI) shows hypersignal on T2/FLAIR the pontine tegmentum, extending to medial aspect middle cerebellar peduncles dentate nuclei. Spine MRI alterations suggestive extensive longitudinal myelitis, signal alteration at pons medulla. Acute disseminated encephalomyelitis (ADEM) suspected. Treatment methylprednisolone 30mg/kg/day performed, showing progressive improvement. Discharged for outpatient follow-up neuropediatric, physical therapy prednisolone 1mg/kg/day 30 days.

We report a seven year old male with measles associated acute disseminated encephalomyelitis (ADEM) despite having received measles vaccination in infancy. The diagnosis was based on serum antimeasles antibodies and MRI brain. The patient was managed with high dose corticosteroids along with supportive measures.
